1. Player card

a. Signed by player with full name as on birth certificate.

b. Player picture attached in appropriate location. Digital pictures printed on home printer not accepted

c. All info completed on player card

d. All info on the card must be original without corrections

2. Birth Certificate

3. New Jersey Youth Soccer form completed with accurate information

4. SAGE sportsmanship pledge form signed by player and parent

5. Medical release notarized

Birth date and player name must be the same on all forms. Make sure all forms are complete and all info is recorded.
